When you want to report an incident, you can create the report in the Vault mobile app. Vault is your private space to create and store records of misconduct that you can send as reports to your company.
Records you create are stored locally on your device. Information you enter remains private and available only to you until you decide to submit the report.
When you send a report, the assigned case team and the company-level administrator can access it.
Disclosure options for reports
When you send a report, you can select how much information about you – and when – is disclosed. By default, Vault offers three ways of sending reports. Depending on your organization’s settings, you may not have access to all the options.
Vault includes the following disclosure settings:
Go Ahead – Select this option when you want to disclose your identity with the report.
Go Anonymous – Select this option to send the report anonymously.
Go Together – Select this option if you want to send the report as part of a larger group. Vault holds your report until another person names the same perpetrator in their report by another person, then sends it as part of a group report.
Regardless of the way you send the report, the team working on your case can always send you direct messages during their investigation. When you select Go Anonymous, messaging the case team does not impact your anonymity.
The case team consists of two individuals designated by your employer, typically members of the HR team.
Create a new report
Create a report and add the necessary information.
In the Vault mobile app, select Create new report.
On the Record type screen, select the type of report you want to create. Select Create.
For more information about each record type, select the ? icon.To add information to your report, select Add an event and answer the related questions. You can also skip the questions and return to them later.
You have an option to add files to your report as evidence. Learn how to add files to Vault reportsAfter you complete the questionnaire, select Review event.
Send a report
When you’re ready, send the report for investigation by the case team.
Open the report and select Ready to submit.
Select one of the available disclosure options.
Before you send the report, you can review the assigned case manager. You also have the option to change them.
To send the report, select Agree & submit.
Next steps
After you send a report, you receive notifications when:
Your report status changes – for example, you'll be notified when the case team opens your report and updates it from "New" to "Read".
A member of the case team sends you a message.
A member of the case team closes your case.
You can get these messages in your Vault app, you can also enable email notifications to receive them directly to your email.
